1. Do I require a doctor’s referral to see a physiotherapist?

No, you do not require a medical note from a doctor to see a physiotherapist. However, sometimes your insurance provider may need to see documentation in order to provide coverage. It is recommended that you review your plan or contact your insurance provider for further information on this topic.

2. Is physiotherapy covered by extended benefits?

Yes, physiotherapy services are commonly included in most extended health plans. However, the amount of coverage varies between providers. It is recommended that you review your benefits package or contact your insurance provider for further information before your first appointment.

3. Do you offer direct billing to extended health plans?

At this time, FreeForm Physio & Pilates offers direct billing to Pacific Blue Cross, Manulife, Canada Life, Sun Life, GroupHealth, Desjardins Insurance and the Chamber of Commerce Group Insurance Plan. If one of these providers are your insurer, please provide your insurance information (member ID number/policy number) before your appointment or bring your insurance information to your first appointment to set up your claim. For all other insurance plans, or for those who would prefer to pay privately, a receipt will be provided following your appointment with all the details needed to receive reimbursement from your provider.

4. Do you accept ICBC claims and WorkSafe BC claims?

FreeForm Physio & Pilates does not currently accept ICBC and WSBC claims.

5. What should I bring to my first appointment?

  • Please bring athletic clothing that is easy to move in and will allow your practitioner to see the injured area (shorts, t/shirt or tank top) - no jeans, no service! (joking - but seriously, don’t wear jeans)

  • If you are participating in clinical Pilates sessions on the reformer please bring a pair of socks to your appointment

  • Any referral notes from your family doctor or other health care practitioners on your team

  • Imaging reports (x-rays, ultrasound, MRI, etc)

  • A go-getter attitude!
